We understand that managing severe mental health challenges requires comprehensive, ongoing support. Our Assertive Community Treatment (ACT) Program provides intensive, personalized care to help individuals achieve stability, independence, and a better quality of life.

This is a voluntary program & clients must be willing to participate.

 
 
 

What is ACT?

ACT is a specialized, evidence-based program designed for individuals with severe and persistent mental illness who need consistent, hands-on support. Unlike traditional therapy models, ACT offers community-based, wraparound care, meaning our team meets clients where they are—at home, in the community, or wherever support is needed.

 
 
 

How We Support You

Our multidisciplinary ACT team includes a mental health professional, case manager, psychiatric nurse, substance use counselor, and peer support specialist who work together to provide:

24/7 Support & Crisis Intervention – Assistance when and where you need it most.
Personalized Treatment Plans – Tailored to your unique needs, goals, and strengths.
Medication Management – Helping you stay on track with prescriptions and care.
Daily Living & Life Skills Coaching – Support with housing, employment, and daily responsibilities.
Integrated Mental Health & Substance Use Treatment – Holistic care for co-occurring conditions.
Community Connection & Social Support – Helping you build relationships and access local resources.

ACT is designed for individuals who:

  • Have a severe mental health condition that affects daily functioning.

  • Have had multiple hospitalizations or crisis interventions.

  • Need consistent, hands-on support to live independently.

  • Would benefit from a team-based, long-term approach to care.
